One Dennis Kucinich donor isn’t filling out her contribution form properly:

The contributions from Jan Van Loan were also made on line. 

In addition, the donor incorrectly entered her name in the fields 

provided for 5 of her contributions. She entered ""Jan Van Loan""

in the first name field and ""Van Loan"" in the last name field, 

resulting in a donor record for ""Jan Van Loan Van Loan."" She 

correctly entered her name for her other contributions, but a 

different donor record was automatically created because of the 

different name and contributions under the two names were not 

aggregated in the database.  She also incorrectly data entered her 

email address in the employer field, resulting in a third unique 

donor record. A refund in the amount of $550 was mailed to her 

on 5/25/12.
